We have been getting a couple of Fatal DB Error
"3219 Invalid Operation"
Any suggestions?

When do these errors occur and in which application (PG Client or Admin)?

Do these errors happen with any regularity or predictability? For example, do they always happen when you try to run a report or send a message, etc?

It does not happen at a specific time, she advised that it happened when they just sent a page and then when the client setting idle.
