Pagegate Group Issues

Since upgrading to 4.0, I am having issue with pages going out to all members of a group. The pages will begin going out to group members and then will stop. There are no errors being logged. Also I can go to each individual user and page them with no issues.

Is the group an "On-call Group"?
What interface are you using to submit messages for the Group?
Is the same interface "enabled" in each recipient's settings?
